Three steps to your first appointment
- Choose a time. Use online booking to pick from the openings in our schedule, or phone us and we will find one with you.
- Complete your paperwork. The new patient forms are online, and they take only a few minutes on a phone or a computer.
- Come in. You will find us on the ground floor at 20 Adelaide Street East, near King Station.
If a tooth is hurting right now, please phone us instead of booking online. We keep room in the day for urgent problems, and we can usually tell you over the phone how soon you should be seen.

The paperwork, and why we ask
We know forms are nobody's favourite part, so here is why they matter. They cover three things. Your medical history, because conditions such as diabetes or a heart problem, and medicines such as blood thinners, change what is safe to do. Your dental history, so we know what has been tried. And your insurance, so the claim can go straight to your insurer.
Completing them at home gives you time to look up a medication name or a policy number without a queue behind you. If you would sooner do them here, that is fine. Come a little early and we will help you get set up.
Bringing your history with you
Recent x-rays and notes from the office you used to attend are useful to us, and they are yours to move. Sign our consent form and we will write to that office for you. Offices generally send them within a few days, so it helps to start before your appointment and not on the morning of it.
Please also bring your insurance card or policy details, your Canadian Dental Care Plan member card if you have one, and, for students, your school plan information. Our student plans page lists the schools whose plans we work with.
How the visit itself goes
You will meet the dentist, talk through why you have come, and have a full examination. We take x-rays when there is a reason for them, and we will tell you what it is. Dr. Keem sees patients in English and Korean, and Dr. Zhao in English and Mandarin, so please say which you prefer when you book.
At the end, we will go over what we saw and what your choices are. A written plan sets out each step and its fee, and what your insurance is expected to pay. The decision about what happens next, and when, is always yours.
What it will cost
Cost is a fair thing to wonder about. Every patient is charged from the same Ontario Dental Association fee guide, and we send the claim to your insurer ourselves. What is left for you depends on your plan. See fees to learn how we estimate your share ahead of time.
